Before You Start ...

A few things you should know to ensure a successful registration:

You will be asked to create a username and password to register. You will be required to login with your username and password to schedule a heart screening appointment for other family members, as well as to access your results online.

Additionally, you will be asked to provide personal information about yourself and/or your child, including:

  • Name
  • Mailing Address (to receive results)
  • Phone Number
  • Email address (to receive appointment and payment confirmations)
  • Date of birth
  • Height and Weight (for BMI calculation)
  • Gender
  • Ethnicity
  • Personal and Family History cardiac questionnaire (for the person being screened)

Finally, you will also be asked to provide consent for the heart screening, and to provide a Visa or MasterCard for payment.  We only accept Visa or Mastercard at this time.  We do not accept checks or any other form of payment.

You should have all of the above information available when you start the registration process. You will not be able to save your answers until payment is complete. If you are unable to complete the registration process, you will have to start the registration process from the beginning.

Cancellation policy: HeartScreen America will refund 100% of the screening cost for appointments cancelled more than 48 hours prior to the appointment time. Refunds will not be made if requested within 48 hours of the scheduled screening.

All missed appointments will be billed as if services were rendered.
